Need help with Poll colors/code

In our site, we have a free web poll service in the right side.
I am running .php this time in the site. The poll has radio buttons, the background color surrounds everything correctly, except the radio buttons. They are square with white, not round.

When I look at it in Dreamweaver MX, the poll selections are correct color, but when it publishes, it is not right.

The poll code it'self seems to display correctly could it be maybe that you missed a close tag somewhere before this code

Just thinking I also noticed or did not see a <!DOCTYPE>
