This (http://www.hotscripts.com/PHP/Scripts_and_Programs/Mailing_List_Managers/index.html) page on HotScripts made me chuckle. I think five of the mailing list scripts are the "most advanced."
And why are the ones I want to try the ones that are always dead? Like MadDog (http://www.hotscripts.com/Detailed/30934.html).

Two good formmails that I have used are
Form to email with attachment (http://obiewebsite.sourceforge.net/obie.php?Form_to_email_with_attachment) Very easy to configure as well as skin to fit the layout of your site. I used it on a previous site where I needed to recieve attachments, but didn't want to give out my email address.
Sending HTML email
Supports one attachment file upload
HTML simple editor on Form
Configuration with Max file size, Banned file extensions (for server security)
AlphaMail (http://www.codebrain.com/perl/alphamail/index.html) is the mailer I currently use and is also very easy to configure.
