I have a D-Link DI-713P wireless broadband router. Just recently, it stopped giving me internet access. I followed the instructions on D-Link's website to reset the router back to factory specs, then power cycled the computer, router and cable modem and still no internet.
When I hook the PCI Ethernet card directly to the modem (without the router), internet works fine.
Could it be a firmware upgrade needed or possibly a bad router? Please help.

can you get into the router's web interface when the computer is connected to it? When the computer is connected to the router, does it get an IP address? (should be a 192.XXXX address).
When rebooting everything, make sure things are powered up in this order:
1. Modem
wait 30 seconds
2. router
3. computer

I did just as you instructed and cannot get an IP address or to the web-based configuration tool.
I get this error - Media Disconnected
All RJ-45 connections are correct. All cables are secure.
Why is this happening?
media disconnected means that the device can't detect a keepalive signal on the cable. This means that there is something physically wrong with your connection...either the cable is bad (broken/emi/miswired), the end connectors are bad (rj-45, maybe not crimped properly, or broken), or a device on one side or the other is broken and not relaying signal data properly.
Chances are it's your cable. Verify it's plugged in properly. Assuming it is (you say you've checked it) you may wish to acquire another patch cable and try that. If that fails, try running a patch cable from the cable modem directly to a pc and see if it works. If it does, the router is having issues. If it doesn't, it's probably your cable modem and you should call your ISP.

I have tried 3 different cables from the router to the NIC and the router to the cable modem. No combination produces the desired results.

So, the router DID work and now stopped working? You get Internet access when you do NOT use the router, but when you try the router, you cannot get Internet. What OS and browser are you using? If using Windows OS and Internet Explorer, your ISP can help you better since they have more experience with them. Check your browser Tools > Internet Options > Connections and click LAN Settings. Be sure to have Automatically Detect Settings checked.
You can look at the manual or visit the website for the manufacturer of your router for troubleshooting tips. Try to resolve the problem yourself before contacting your ISP since they will probably charge you for the visit if the problem is not related to THEIR service.
If you recently installed software, upgraded your OS or installed Windows updates, your computer may have had its settings changed. Windows can be quirky that way. Of course, you have already tested most hardware issues first, so checking the software is recommended.

I should have mentioned that if you have a firewall program installed such as Zone Alarm, you should disable or remove it. Your router probably has a built-in firewall that works better than those programs anyway.
Check the router manufacturer website for the correct network drivers for your computer. If you have another computer to test your LAN, you may be able to see if this is a router problem or not. If you can get another computer online, then you probably have a software problem.
